How referrals work

  1. Send the referral

Contact our Service Manager directly or complete the referral form below.

  2. Free initial assessment

We aim to arrange a free, no-obligation initial assessment within 24 hours. This gives us an opportunity to meet the individual, understand their needs, preferences and support requirements, and explore how our service can best support them.

  3. Assessment report

Following the assessment, we provide a detailed report outlining the individual's needs, recommended support arrangements and a clear breakdown of costs, ready for submission to your funding panel for consideration.

Questions about referrals

What is supported living?

People live in an ordinary shared house in the community, with 24-hour support from our staff, including sleep-in staff at night. Housing and care are arranged under separate agreements. The aim is for each person to build skills and confidence, and move on to more independent living when they're ready.

Who can make a referral?

Social workers, care coordinators, placement and brokerage teams, and hospital discharge teams can all refer. Families and friends are welcome to get in touch too, by phone or through the same form.

Do you accept referrals from outside Hillingdon?

Yes. Our homes are in Hayes and Uxbridge, in the London Borough of Hillingdon, and we consider referrals from other areas too.
