How we use your information
This notice explains what happens to the information you give us through this website.
Who we are
Panacea Care Limited (company number 07999732) is the data controller for information collected through this website. Our registered office is 13 Wood End Green Road, Hayes, Middlesex UB3 2SB. You can contact us at info@panaceacare.co.uk or on 020 8848 9201.
What we collect
When you send a referral, we collect the details you enter about you: your name, role, team and organisation, email address and phone number, and how you heard about us. We also collect the general details you give about the referral: the referring area, where the person is now, the timescale, expected funding and a summary of support needs.
Please do not include the name, date of birth, NHS number or medical details of the person you are enquiring about. Once we are in touch, we will agree a secure way to share that information.
Why we use it
We use your details only to respond to your enquiry and, if you ask us to, to begin the referral and assessment process. Our lawful basis is our legitimate interest in replying to enquiries, and taking steps at your request before any care arrangement is agreed.
Who can see it
Your enquiry is seen by our service manager and the staff who need it to respond. The website is hosted by Netlify, and the referral form is processed by Formspree, which emails it to us. Both act on our behalf. We do not sell your information or share it for marketing.
How long we keep it
We keep enquiry details only for as long as we need them to deal with your enquiry. If it leads to a placement, the information becomes part of the care record and is kept in line with our records policy.
Cookies
This website does not use tracking or advertising cookies. The Care Quality Commission rating shown at the bottom of each page is provided by the CQC and loaded from their website.
Your rights
You have the right to ask for a copy of your information, to have it corrected or deleted, and to object to how we use it. To make a request, contact us using the details above. If you are unhappy with how we have handled your information, you can complain to the Information Commissioner's Office at ico.org.uk.
